Please read before submitting Alaska Workforce Safety Week 2027 Training Provider Agreement This agreement is between Alaska Safety Alliance (“ASA”) and the organization submitting this application (“Provider”). It applies to each training listed in the application once ASA approves it for Alaska Workforce Safety Week, February 8–13, 2027. 1. Delivering the trainingOnce ASA approves the application, Provider agrees to deliver each approved training as described in the application, including its content, date, time, location or virtual format, price, capacity and any certificate offered. Provider will use qualified instructors. Any change after approval must be sent to ASA by email; ASA will update the listing. 2. Registration and priceAll registration takes place through ASA’s Safety Week registration on alaskasafetyalliance.org. Provider will not charge attendees anything beyond the listed price. Provider accepts up to 2 registrations above the listed capacity to allow for no-shows. If everyone attends, Provider will work with ASA to seat the extra attendees or arrange a full refund. 3. Cancellation by ProviderProvider may cancel a training by notifying ASA in writing at least 3 days before the training’s scheduled start. This includes cancelling a paid training that has not reached the minimum number of registrations it needs to run. Where weather, an emergency or unsafe conditions make the 3-day notice impossible, Provider will notify ASA as soon as possible. ASA will notify all registered attendees. 4. RefundsIf Provider cancels a training for any reason, every registered attendee receives a full refund of their registration fee. Provider will not be paid for a cancelled training and will return to ASA any amount already paid for it. 5. Payment for discounted trainingsASA collects all registration fees for discounted trainings through its website and pays Provider the registration fees received for each training after that training has been delivered. ASA pays any card processing fees. Payment is made by check unless Provider gives ASA its ACH (direct deposit) information. ASA requires a completed IRS Form W-9 from every provider it pays, before payment is issued. 6. InsuranceProvider maintains its own Commercial General Liability insurance, Professional Liability insurance and, if it has employees, Workers’ Compensation insurance through the date of its last training. Provider has supplied a current Certificate of Insurance and will tell ASA promptly if coverage lapses or changes. 7. Safety and complianceProvider is responsible for its training site or virtual session and for providing a safe learning environment that follows applicable laws and safety standards. Provider will give accurate information about prerequisites, required PPE and accessibility. Where attendees under 18 are admitted, Provider will provide appropriate supervision. 8. Attendance reportingWithin 5 business days after each training, Provider will submit ASA’s attendance report, including the number attended, walk-ins, no-shows and certificates issued, along with the marked-up roster. 9. Attendee informationRosters and attendee information are confidential. Provider will use them only to deliver the training, issue certificates and handle related follow-up, and will not share them or use them for marketing without the attendee’s consent. 10. PromotionProvider allows ASA to use its organization name, logo and training details to promote Alaska Workforce Safety Week, including on ASA’s website and map, in social media, print and sponsor reports. Provider may promote its own Safety Week trainings and will direct registrations to ASA’s registration page. 11. Independent organizationsProvider is an independent organization and is not an employee, agent or partner of ASA. Provider is responsible for its instructors, staff and training activities, and agrees to hold ASA harmless from claims arising from Provider’s trainings. 12.
